Job Location

This position will practice in Duke University Hospital, inpatient unit (9200) as well as the outpatient Duke Oncology Treatment Center (OTC) located in the North Pavilion.

Job Summary

The OTC APP will provide direct patient care in the inpatient setting to patients during the pre-, peri- and post-chemotherapy periods in collaboration with Hematology/Oncology attending physicians and nursing staff. The age-specific scope of practice is between 18 yrs and end of life.

Job Duties and Responsibilities:

In collaboration with Dr. Hope Uronis (supervising physician and OTC medical director) as well as OTC Leadership, the Advanced Practice Provider’s primary focus is to augment the care of patients receiving care and treatment in the OTC. This includes:

  • Oversight of emergency management of patients
    • Hypersensitivity protocol
    • Code Blue/ 115 first response
  • Assessment of acutely ill patients awaiting admission to the hospital
    • Additional orders for symptom management as needed
    • Collaboration with inpatient medical team for handoff
  • Prescription of medications for symptom management
    • Additional orders as needed
  • Address lab values if outside of regimen parameters to determine if patient can receive treatment
    • Order supplements as needed based on lab values
  • Rounding in the OTC throughout the day, connecting with the Disease-Based Group (DBG) Team Leads and Charge Nurses
  • Perform outpatient procedures in coordination with patient’s primary oncology provider team
  • Assessment of patients as needed prior to treatment in coordination with patient’s primary oncology provider team
  • Daily rounding on patients receiving chemotherapy and blood products in the OTC.
  • Evaluation of patients for needs including, but not limited to, fluids, electrolyte replacement, antibiotics, pain medication, anti-emetics, admission to inpatient status, etc.
  • Assessing and clearing patients for treatment on behalf of their primary treatment team on a limited, case by case basis, as OTC workflow allows.
  • Overseeing hypersensitivity reactions and other acute events.
  • Yearly review of hypersensitivity protocol.
  • Proper documentation of all interventions and therapies into the medical record.
  • Communicating with the patients’ primary oncologist and oncology teams such that all parties involved in their care are aware of the most up to date information about the patients.
  • Perform minor procedures such as simple superficial suture placement and removal, EKG interpretation, preliminary image interpretation, ommaya access with chemotherapy and other procedures within competency and personal scope of practice, as needed in the OTC.
  • Actively participate in discussions and decision making that affect the clinical policies and procedures of the OTC.
  • Attendance at educational events when time permits.
  • Education and in-services for staff regarding oncology topics, new procedures, disease processes, etc., when time permits.
  • Assessment and management of patients in Acute Care Clinic for urgent care needs, when they are unable to see their oncologist, but that would be preferable not to go to the ER.
  • Meetings with my supervising physician every six months per Collaborative Practice Agreement.
  • Responsible for accurate charge capture/billing as well as documentation of care provided in the patient’s medical record.
  • Responsible for communicating care provided in the OTC with the patient’s primary oncology provider team.

Job Eligibility Requirements

Education

NP: Graduate of MSN or DNP accredited Nurse Practitioner Program

PA: Graduate of ARC accredited Master’s Degree or Doctoral Degree Physician Assistant Program

Certification

NP: Any NP certification that treats adults (FNP, ANP, AGNP, AGACNP, etc.)

PA: PA-C

Licensure

NP: Must have unrestricted RN licensure issued by or recognized by the NCBON with authorization to practice as a NP

PA: Must have unrestricted license issued by NCMB with authorization to practice as PA

Both: Federal and State DEA independent license with prescriptive authority

Experience

APP experience is preferred, ideally Oncology/Hematologic Malignancy experience, but recent or new grads, graduating by May 2024, welcome to apply.

Job Hours

Full-Time, 40 hour week, Monday – Friday, Shifts rotate with 2 other providers to provide coverage during clinic hours, 8am – 8:30am
