Job Description

Work Environment:

Outpatient education is provided primarily in an office and classroom; inpatient education in provided in the clinical setting.

Supervisory Responsibilities:

            None

Financial Responsibilities:

            None

Essential functions:

  • Assesses the needs of patients referred for diabetes education.
  • Develops a plan of care that prescribes interventions to attain expected outcomes.
  • Implements and/or coordinates Diabetes Education.
  • Provides instruction to patients and family members about life skills self-management; provides refresher education as needed.
  • Provides instruction to staff about diabetes management issues.
  • Evaluates the effectiveness of diabetes education toward attainment of outcomes.
  • Promotes community outreach and participates in health screenings.
  • Performs other miscellaneous and related duties as assigned or requested.

Physical Requirements:

Physical requirements are primarily related to the essential functions of any job.  In order to perform the essential functions of this position, you must be able to:

  • Stand, walk, sit, use hands, reach, stoop, talk, hear and smell.
  • Lift or exert energy up to 25 pounds 50% of the time, 25 pounds 25% of the time and 50 pounds 25% of the time.
  • Maintains close vision [clear vision at 20 inches or less], distant vision [clear vision at 20 feet or more], color vision [ability to identify and distinguish colors], peripheral vision [ability to observe an area that can be seen move up/down or left/right] and depth perception [three-dimensional vision, ability to judge distances].

Mental Requirements:

Must possess the ability to:

  • read, analyze and interpret complex scientific, clinical or business journals, financial reports or legal documents. 
  • write reports, correspondence and policies.
  • respond to sensitive inquiries or complaints from guests, regulatory agencies, staff and members of the business community.
  • effectively present information to administration, large public groups, and nursing/medical staff.
  • define problems, collect data, establish facts and draw valid conclusions.
  • deal with concrete and abstract concepts and interpret verbal, non-verbal and written instructions.

Age Groups:

This position primarily interacts with adults and aging adults; but provides education to the diabetic patient [and his/her family member(s)] of all age groups.

Additional Job Description

Current RN license to practice in the state of South Carolina or compact state required.

A minimum of three years of experience required.

BSN and CDE preferred.
