Job Description Summary
The MUSC Health Medical Scheduling Coordinator is responsible for ensuring patients identified as high or medium risk for hospital readmission are scheduled for a post-discharge Transition Care Management face-to-face follow-up appointment with the appropriate primary care provider or specialist prior to being discharged from the acute care setting. This includes assisting with other patient needs, and confirming/facilitating the patient successfully presenting for their scheduled post-acute care appointment. The Medical Scheduling Coordinator will assist patients, patients’ families, physicians, and others to resolve patient-related issues.
This position will work under the direction of the System Case Management and will have a dual reporting structure and will work closely with the Patient Access Center, Population Health, Case Management and Care Transitions, Primary Care teams, and hospital leadership.Entity

Job Description
Job Duties and Responsibilities:
Scheduling of post-discharge follow up appointments
· Works directly with patients identified as high or medium risk for readmission, and their family/care giver(s), before discharge from acute care to schedule a post discharge follow up visit with the appropriate primary care provider, specialist, or APP.
· Utilizes electronic record systems to access, enter and edit patient information.
· Utilizes knowledge of physician practices to transition care management to schedule patients designated as high or medium risk for readmission for a post-discharge follow-up appointment with the appropriate provider within 7 days of discharge from acute care.
· Communicates information to patients and families regarding preparation for post-discharge visits, laboratory tests, examinations, and procedures.
· Resolves any schedule conflicts. Assures all patient correspondence (orders, medication list, history and physical, etc.) is transmitted to the correct areas in a timely manner to streamline patient processing.
· Other duties as assigned.
Communication
· Obtains medical records from clinics and outside hospitals upon team request to coordinate and progress inpatient care.
· Communicates and rounds with multidisciplinary inpatient hospital teams routinely.
· Proactively communicates with providers’ clinic staff to schedule/reschedule patients to efficiently utilize resources and meet patient scheduling needs. Coordinate and communicate with any community resources as needed.
· Acts as a liaison for patients and their families to include appointment scheduling, correspondence regarding patient expectations, and follow-up.
Additional Job Description
Required Minimum Training and Education:
· The position requires a high school diploma or equivalent, 2 years college or equivalent work experience, and experience working in Epic is preferred.
· Knowledge of Medicare Transitional Care Management of moderate and high medical complexity post-discharge face-to-face provider visit requirements is preferred.
· Requires excellent oral and written communication skills, and strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
· Must be proficient with computers.
· Ability to establish cooperative relationships and sensitive to the needs, concerns and safety of patients and visitors/families of diverse cultural orientation.
· Must be able to work independently with minimal supervision.
Required Licensure, Certifications, Registrations:
· HIPAA certification is to be completed upon hire and ongoing as required.
· Successful completion of all required training courses to include: Electronic Patient Records (EPR), Epic scheduling, ORMIS scheduling, and any other software systems.
