Job Description

Fully grant funded, continuation dependent on renewal.

Community Outreach Coordinator

Position will work with the Division of Community Health’s Benefits Enrollment Center (BEC). This grant funded program from the National Council on Aging assists with benefit uptake and resource referral for Medicare Beneficiaries, and older adults with disabilities. Position will work directly with clients, navigating benefit eligibility and application processes. The coordinator will be provided training in policy and procedure in federal, state and local benefit programs. Work will include conducting outreach to develop relationships with partner organizations in Durham, Person, Granville and Wake counties. Coordinator will help in development of supplemental outreach materials, and may assist with monthly programmatic reports.

Minimum Qualifications

Education

Work requires an educational background normally equivalent to a high school education. Nurse Assistant Training preferred but not required.

Experience

Work requires one year of experience in a position involving OR AN EQUIVALENT COMBINATION OFRELEVANT EDUCATION AND/OR EXPERIENCE required. patient/public contact. Valid North Carolina driver's license with a good driving record is
