General Statement of Duties
Closing Date: This position closes on Monday, December 9, 2024.
Provides screening, counseling, case management, training or other specific treatment or rehabilitation activities as directed for the Community Support Team; does related work as required.
Essential Duties
This position provides case management services to include linking, monitoring, and coordinating. Assesses client service needs, capabilities, and appropriateness for services; presents options and services based upon a needs assessment and client preference. Requires extensive knowledge of Medicaid Waiver and SPO to ensure appropriate provision and documentation of services. Develops a person-centered plan of care and services appropriate to the individual, reviews and updates service plans. Makes referrals and linkages to appropriate agencies for services, coordinates client services and treatment with multiple service providers and agencies. Evaluates the quality of services provided and monitors the client's well-being; evaluates client's environment for safety. Serves as liaison to public agencies and provides information regarding Authority programs and services. Prepares a variety of reports; prepares and maintains client records. Attends meetings, staffing's, and conferences as they relate to client, staff and program needs.
Knowledge of principles, practices and methods of treatment and/or rehabilitation of individuals with an intellectual disability or mental illness; some knowledge of mental health and/or intellectual disabilities and related problems; good interpersonal skills which include the ability to empathize, demonstrate respect, communicate, reflect feelings and confront constructively; ability to relate freely to and work effectively with clients and professionals from all levels of the community; adherence to basic ethics appropriate for any helping relationship; ability to write concisely and coherently; demonstrated ability to use good judgment.
Minimum Qualifications
A minimum of a bachelor's degree in a human service field including, but not limited to, sociology, social work, special education, counseling, or psychology. An advanced degree in a related field fulfills the requirements if a qualified candidate does not have a bachelor’s degree in a related field area. At least 1-year documented experience working directly with individuals who have developmental/intellectual disabilities.
Additional Information
Possession of a valid Virginia driver's license is required. TB Screening and Child Abuse Registry Check required, upon hire.
Desire experience in the provision of Waiver Case Management and SPO Case Management. Prefer at least one year experience working with individuals with intellectual disabilities.
This position is part of an approved Career Development Plan (CDP) and offers career development opportunities within the position based on the performance, qualifications and experience of the incumbent.

Expected salary is at the lower-mid range of the posted salary scale.
