General Statement of Duties
Closing Date: This position closes, Monday, December 9, 2024.
Performs targeted outreach to educate and engage target populations in non-emergent resources and services, identifies opportunities to proactively engage with the County’s at-risk residents, develops intervention strategies tailored to meet individual needs by providing person-centered and trauma informed case management; identifies opportunities for early interventions; develops case management plans that coordinate services across multiple County agencies and community resources and services to reduce over-utilization of 911 services for non-emergency needs; performs other duties as assigned.
Essential Duties
Join our CARE Team to make a difference in the community by leading outreach and case management efforts to support at-risk residents. Use your expertise in case coordination and community partnerships to reduce 911 overutilization and enhance service delivery across multiple County agencies and resources.
Core responsibilities:
- Supervise and guide the CARE Team, including civilian and sworn staff, ensuring effective service delivery and communication.
- Evaluate and improve CARE Team operations to increase efficiency and community impact.
- Develop and implement personalized care plans, connecting individuals with County and community resources.
- Identify trends, engage with at-risk residents, and educate stakeholders about available services and interventions.
- Build and maintain relationships with local agencies to strengthen referrals and expand program reach.
- Analyze program data to assess outcomes, identify trends, and provide insights to leadership.
- Represent the CARE Team in community meetings and initiatives, advocating for vulnerable populations.
The successful candidate will have:
- Strong background in social work, case management, or community-based services.
- Excellent knowledge of County agencies, resources, and protocols for protected health information.
- Strong communication skills, including the ability to engage with diverse communities and deliver presentations.
- Ability to analyze data, develop intervention strategies, and create actionable care plans.
- Flexibility to work in various settings, including homes and offices, with some travel required.
- Ability to manage a budget and ensure program resources are used efficiently.
- Proficiency in computer software and applications for task completion and communication.
Minimum Qualifications
Four (4) year degree in a relevant field such as social work, relevant master's level degree preferred; and five (5) years of responsible directly related work experience, preferably with prior supervisory experience; OR: Any equivalent combination of experience and training which provides the necessary knowledge, skills and abilities.
