The MUSC Institute for Neuroscience Discovery (MIND) Nurse Navigator will
• Assist in coordinating the care of patients accepted into the MIND program. Coordination activities include working with MUSC Health social work staff to transition care of MIND patients to rehabilitation facilities and/or home. Other activities include rounding on wards with MIND patients, identifying new patients appropriate for the MIND program, collection of clinical data, longitudinal follow up of MIND patients and management of other MIND team members. The MIND Nurse Navigator reports directly to the MIND Executive Director (ED).
• Additionally, the Nurse Navigator communicates with referring physicians and collaborates with members of the clinical team, including the attending faculty and staff to help develop an appropriate plan of care for patients and their caregivers. Under the supervision of the MIND ED, the Nurse Navigator will use advanced clinical pathway guidelines to provide individualized, goal-directed quality care to patients and their caregivers. Some direct patient care, including but not limited to MUSC facilities as well as patients’ homes, will be expected.

Job Description

The MUSC Institute for Neuroscience Discovery (MIND) Nurse Navigator will

  • Assist in coordinating the care of patients accepted into the MIND program. Coordination activities include working with MUSC Health social work staff to transition care of MIND patients to rehabilitation facilities and/or home. Other activities include rounding on wards with MIND patients, identifying new patients appropriate for the MIND program, collection of clinical data, longitudinal followup of MIND patients and management of other MIND team members. The MIND Nurse Navigator reports directly to the MIND Executive Director (ED).

Additionally, the Nurse Navigator communicates with referring physicians and collaborates with members of the clinical team, including the attending faculty and staff to help develop an appropriate plan of care for patients and their caregivers. Under the supervision of the MIND ED, the Nurse Navigator will use advanced clinical pathway guidelines to provide individualized, goal-directed quality care to patients and their caregivers. Some direct patient care, including but not limited to MUSC facilities as well as patients’ homes, will be expected.

Job Duties and Responsibilities:

  • Coordinates care for MIND patients from initial access, consult or referral. Functions as a liaison between Scheduling and Clinical staff to facilitate the efficient flow of patients throughout the system (outpatient and inpatient). 20%
  • Reviews appointments in Epic prior to clinics (including TeleHealth clinics) to manage services needed prior to clinic, W/U packets, medications, ancillary services, etc. Reviews new patient and follow-up patient test results (imaging, labs, etc.) with provider and implements any plans discussed. Serves as a liaison between referring physicians, inpatient services, outpatient services, residents, nurses and providers. Provides coordination between disciplines to ensure that the care of patients and their caregivers is efficient, consistent and timely. 20%
  • Confers with referring physicians regarding need, urgency, appropriateness of appointment/clinic visits, ancillaries as necessary. Assesses and reviews referral records to assist with scheduling patients in a timely fashion. Establishes a plan of care through collaboration with clinical team members and coordinates follow up and subsequent care with multiple departments and referral networks. 15%
  • Work with clinical support staff to maximize efficiency in clinic scheduling, follow up care, and patient messaging. Documents all patient care activities in Epic including disposition of patient telephone calls, medication lists, etc. Generates radiology requests in Epic; updates medicines as needed in Epic. Responds to MIND patient messages and calls in a timely manner. Places orders in Epic. 15%
  • Works with department leadership to measure quality metrics and improve workflow processes in the clinical environment. Triages messages in Epic and tracks appropriate response times. Assists with master template builds for the department and clinic block management. Follows up on patient satisfaction items to improve quality of care in department. Works to improve space utilization and surgical yield metrics. Attends departmental Case & Topic and M&M conferences on a weekly basis. 15%
  • Organizes support group activities, including occasional weekend or evening meetings (in person or remote). Works with MIND’s ED on new clinical initiatives. Prepares monthly reports on pre-cert, appointment scheduling, message turn-around, and other data for submission to department leadership. Complete other duties as necessary. 15%

Degree of Supervision:

This position will work under direct supervision of the Executive Director of MIND. The Nurse Navigator will work in numerous clinical care environments, including but not limited to MUSC Main Hospital, MUSC Ambulatory clinics, MUSC Neurologic Rehabilitation Institute and on some occasions patients’ homes. The Nurse Navigator will be expected to deal effectively with stressful situations involving patient care after a brief period of orientation.

Additional Job Description

Required Minimum Training and Education:

Minimum qualifications include two years experience as a Registered Nurse in addition to licensure as an RN by the State of South Carolina Board of Nursing. Prior experience caring for Neurological and/or Neurosurgical patients also preferred but not required. Also requires certification in Basic Life Support. Organizational ability, excellent interpersonal skills, intelligence and maturity to handle work deadlines and stress while maintaining a high degree of initiative and sound judgement. Ability to honor confidentiality. Ability to effectively communicate both orally and in writing. Must possess an ability to establish cooperative working relationships with diverse groups and individuals, medical staff and other health care disciplines and a thorough understanding of the consultative process.

Required Licensure, Certifications, Registrations:

  • Licensure as an RN by the State of South Carolina Board of Nursing.
  • Basic Life Support

Physical Requirements:

Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods of time. Requires occasional lifting and carrying items weighing up to 50 lbs. Requires corrected vision and hearing to normal range. Absence of color-blindness. Requires working under stressful conditions or working irregular hours. Requires some exposure to communicable diseases and body fluids. Requires frequent bending, stooping, reaching, pushing, and pulling. Freedom from mental disorders, which would interfere with performance of duties. Speaking ability sufficient to communicate effectively with other individuals in person or over the telephone. Must be able to communicate effectively in writing. Ability to operate patient care equipment.
