The Wound Care Nurse reports to the Nursing Director Under limited supervision, the Wound Care Nurse provides an in-depth plan of care for patients in a specialty area of wounds, ostomies, and urinary continence for all ages (neonate to adult) throughout the organization. This role coordinates with nursing management and collaborates with other healthcare providers to implement programmatic changes and update clinical practice based on current research and changes in the age and level of development of patients, with consideration of the needs of patients’ families.
Additional Duties and Responsibilities: Communicate daily with CNAs, charge nurses, and supervisors related to patient skin conditions, changes in treatment orders, preventive skin care needs, etc. as indicated. Follow physician’s orders to carry out all treatments as ordered for the entire facility (ointments, dressing changes, etc. Documents each treatment as it is completed. Assures that treatment orders are written and entered in the EMR per physician’s orders. Initiates/assures initiation of appropriate documentation for all skin areas in a timely manner (as soon as noted and initial treatment is begun): Obtains and processes physician’s orders as needed. Enters new skin areas (of any type) in epic Documents measurements and descriptions of all skin areas at least every 7 days on the skin sheet. Collaborates with RN regarding staging and/or classification of skin areas. Collaborates with RN regarding the effectiveness of treatments every 14-30 days if no change in the status of the affected area (reviews with the physician for change in treatment orders). Responsible for performing daily body checks of patients with actual pressure areas and weekly checks for residents with no actual pressure areas, as indicated per skin risk score. Complete/prepare weekly Pressure Ulcer Report turn in to DON and scan to primary nurse consultant each week. Assists unit nurse with body audit of patients on admission or readmission to the facility when on duty Maintain the treatment carts, supplies, and all solutions, ointments, etc. Assure that all containers are properly labeled, dated when opened, and removed after treatment is discontinued. Collaborates as a team member in the development of patient care plans as assigned and assists in implementing the plan on the units.
Assists in providing direct patient care (i.e., personal care, bath, elimination needs, OOB regime, etc.) if needed. Report any noted changes in the patient's condition to the physician and Nursing as necessary. Attends in-service programs when scheduled. Attends staff meetings as assigned. Attends and participates in daily stand-up QI meetings. Encourages both licensed and non-licensed staff to constantly improve the quality of nursing care. Enters nurse's notes in EMR, as necessary. Demonstrate competency using the current clinical software system.
Additional Job Description
Minimum Training and Education: Bachelor’s degree in nursing from an accredited college/university and a minimum of two) (2years' experience of relevant experience with wounds, ostomies, staff development, and quality metrics. supervisory experience preferred. Extensive knowledge and skill in the patient population, health care trends, community and regional resources, and service availability to these populations. An ability to establish working relationships with diverse groups and individuals, medical staff, and other healthcare disciplines. Considerable knowledge of research and investigative methodologies. Considerable knowledge and understanding of the consultative process. Knowledge of consultative and operational processes.
Required Licensure, Certifications: Licensure as a registered nurse by the SC Board of Nursing or a compact state is required. Current Basic Life Support (BLS) with the American Heart Association (AHA)
