Job Description Summary

The Nontuberculous Mycobacteria (NTM) and Bronchiectasis Licensed Master Social Worker assists patients and their families in making psychosocial adaptations specific to their diagnosis while facilitating eligibility determination for social programs and financial resources. The social worker will apply optimal utilization of health care services and community resources. The social worker will assess, plan, implement, deliver, monitor and evaluate the options and services required to address a patient's individualized plan of care in collaboration with the interdisciplinary team to achieve optimal outcomes.
Assist patients and their families in the NTM Clinic in making psychosocial adaptations specific to their diagnosis, while facilitating eligibility determination for social programs and financial resources. Apply optimal utilization of health care services and community resources in accordance with the principles of human growth and development. To provide social work services to MUSC patients and families utilizing a collaborative approach. The social worker will assess, plan, implement, deliver, monitor and evaluate the options and services required to address a patient's individualized plan of care in collaboration with the interdisciplinary team to achieve optimal outcomes.

Job Responsibility:  Develops and Initiates the Psychosocial Plan of Care

  • Conducts a psychosocial assessment, evaluating patient psychosocial risk factors. Demonstrates problem solving and critical thinking through reassessment of the patient’s plan of care, adjusting goals and interventions as needed.  Evidenced by documentation in the medical record and/or team observation/feedback.
  • Based on assessment, communicates recommendations with the patient, family and team to support a collaborative patient-centered approach. Ensures effective communication through patient handoffs with interdisciplinary team as evidenced by documentation in the medical record and/or team observation/feedback.
  • Develops an action plan with the patient, family and team taking into consideration variables that impact the patient’s plan of care to include social, emotional, physical or environmental factors as evidenced by documentation in the medical record and/or team observation/feedback.
  • Assists patients and families with navigating the complex healthcare system, accessing financial aid, and assisting with insurance barriers/issues.
  • Facilitates patient education and self-management strategies to improve adherence to treatment plans.
  • Assesses patients for barriers to care otherwise known as the Social Determinants of Health (SDoH) on an annual basis, minimally.
  • Makes regular telephonic contact with patients who are moderate to high risk for readmission.

Additional Job Description

Education: Master's degree in social work (MSW)and LMSW-Licensed Master Social Worker.
