Job Description Summary

Provides solid organ transplant social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions which may include initial and annual assessments, identifying mental health disorders, brief behavioral health interventions, information and referral, during hospitalization and/or outpatient treatment. May provide educational support to staff. Participates in quality meetings and committees. Collaborates with the multi-disciplinary team. Knowledgeable in ESRD Medicare and able to assist patients with maintaining adequate health care coverage. Maintains a positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues and identify and address psychosocial barriers. Master’s degree in Social Work and licensure required. Minimum of one year of current experience in a clinic, hospital, medical setting, or health care field preferred. This position participates in an on call rotation of the Organ Transplant Social Work Team. Experience with transplant patients or dialysis patients preferred.
